# Stem cells weekly source review - September 21, 2026 Decision: substantive public update warranted. New long-term human follow-up, new MS biomarker analyses and a company-reported early-phase COPD milestone were identified. An important earlier Japanese regulatory development was also verified and added as a dated clarification, NOT presented as news from this week. Article: https://managingexpectations.net/blog/articles/stem-cells-evidence-costs-risks-global-guide.html Previous note: https://managingexpectations.net/research/health/stem-cells-weekly-update-2026-09-14.md Access date: 2026-09-21 (PDT). Main literature entry-date window: September 14-21, inclusive; publication dates checked separately. This is targeted monitoring, not a systematic review or certification that no other changes occurred. ## Findings selected for the article ### 1. Ten-year retinal graft follow-up: promising, single-patient human evidence Daiki Sakai et al. "Ten-Year Survival of First-in-Human Autologous Induced Pluripotent Stem Cell-Derived Retinal Pigment Epithelium Sheet Transplantation." Ophthalmology Retina, electronic publication September 16, 2026. DOI: 10.1016/j.oret.2026.09.005. PMID 42749023. https://pubmed.ncbi.nlm.nih.gov/42749023/ Primary abstract retrieved through NCBI EFetch XML. Design explicitly states "Follow-up of a single patient." Autologous iPSC-derived retinal pigment epithelium sheets were transplanted in 2014 for neovascular age-related macular degeneration. The report describes ten-year imaging follow-up, persistent pigmentation consistent with the graft, and preservation of adjacent retinal/choroidal structures. The publication date is not the procedure date. What changes: meaningful long-term structural persistence evidence, useful in a field with durability and abnormal-growth concerns. What it does NOT show: population-level effectiveness, a reliable rate of rare adverse events, a broadly proven vision-restoration treatment, or a commercial-clinic endorsement. Do not translate "survival" in the title into a patient-survival benefit or a cure. Evidence label: promising/under study, one patient. ### 2. MS biomarker changes do not establish clinical benefit Violaine K. Harris et al. "Effect of intrathecal injection of mesenchymal stem cell-neural progenitors on cerebrospinal fluid biomarkers in progressive multiple sclerosis." Stem Cells Translational Medicine, September 18, 2026;15(10):szag083. DOI: 10.1093/stcltm/szag083. PMID 42754254. https://pubmed.ncbi.nlm.nih.gov/42754254/ Primary abstract retrieved via NCBI. Analysis uses cohorts from a phase 2 trial (n=50) and expanded access (n=43). Four cerebrospinal-fluid markers changed: CCL2, MMP9, SCF and CHIT1. Neurofilament light and GFAP did not change. The abstract describes pharmacodynamic response, not proof of disability reversal or durable repair. Irene Schiavetti et al. "Effect of mesenchymal stem cells on peripheral immune cells in multiple sclerosis: results from an international phase 2 clinical trial." Transplantation and Cellular Therapy, September 15, 2026. DOI: 10.1016/j.jtct.2026.08.016. PMID 42744029. https://pubmed.ncbi.nlm.nih.gov/42744029/ Primary abstract retrieved via NCBI. Ancillary immune-cell analysis in 51 participants from MESEMS. Key limit: "the trial did not demonstrate a significant effect on MRI disease activity." Short-term immunological changes and exploratory associations are not a new positive clinical efficacy trial. These are different MSC-derived products/studies; do not pool them as equivalent interventions. Evidence label: under study, mechanistic/biomarker evidence. ### 3. COPD: company-reported permission to study, not approval to market SMSbiotech press release, September 16, 2026, 09:18 ET: https://www.prnewswire.com/news-releases/smsbiotech-receives-fda-study-may-proceed-notification-for-first-us-ind-cleared-inhaled-stem-cell-therapy-in-copd-302880670.html Direct retrieval and web extraction succeeded. The company says FDA issued a "Study May Proceed" notification for a U.S. phase 1b study and targets first U.S. dosing in Q4 2026. It also reports no therapy-related adverse events during the first Australian cohort's 28-day monitoring. Those are sponsor statements, not independently published outcomes. Do not repeat its "first" or disease-modification marketing claims as verified facts. The FDA letter itself was not obtained. Australian SORT-COPD registry: https://clinicaltrials.gov/study/NCT06986070 API: https://clinicaltrials.gov/api/v2/studies/NCT06986070 Direct API retrieval succeeded. Registry last updated August 11, 2026; recruiting; phase 1; non-randomized; sequential; open-label; estimated enrollment 18; hasResults=false. It confirms a separately registered Australian research program, NOT the U.S. IND letter or clinical benefit. This registry change is not new this week. Evidence label: investigational; U.S. milestone company-reported. ### 4. Japan: correction of an overly generic country entry, not a new September approval PMDA review-report index, retrieved directly: https://www.pmda.go.jp/english/review-services/reviews/approved-information/0004.html It lists AMCHEPRY (raguneprocel) and RiHEART as approved in March 2026. English PDF review reports downloaded and text-extracted: - AMCHEPRY: https://www.pmda.go.jp/files/000281617.pdf PDF page 1: conditional/time-limited approval and seven-year duration; surveillance covering all treated patients and specialist institutional requirements. PDF pages 2-3: efficacy information limited and requires further evaluation; indication is improvement of motor symptoms in Parkinson's disease inadequately responsive to conventional pharmacotherapies including levodopa-containing products. Allogeneic iPSC-derived dopaminergic progenitor product. - RiHEART: https://www.pmda.go.jp/files/000281693.pdf PDF pages 1-2: conditional/time-limited approval, all-patient surveillance, seven-year duration. PDF pages 3-4: severe heart failure associated with ischemic cardiomyopathy inadequately responsive to standard care, including pharmacotherapy and invasive interventions; efficacy must continue to be evaluated and confirmed. Allogeneic iPSC-derived cardiomyocyte sheets. The February deliberation/review dates and March approval listings refer to different regulatory steps. Japanese originals prevail over English translations. Evidence label: approved for narrow indication, conditional and time-limited in Japan; not a universal cure, proof of unrestricted access, or approval in Canada/U.S. The article's iPSC category and country table are clarified accordingly. No travel recommendation or dosing guidance is included. ## Required source checks and unchanged conclusions ### FDA Approved cellular/gene therapy list: https://www.fda.gov/vaccines-blood-biologics/cellular-gene-therapy-products/approved-cellular-and-gene-therapy-products Consumer safety page: https://www.fda.gov/vaccines-blood-biologics/consumers-biologics/important-patient-and-consumer-information-about-regenerative-medicine-therapies Web extraction returned list content and the June 3, 2021 consumer warning. Direct retrieval was redirected to FDA abuse-detection pages and failed. Extracted list was partial; no complete list-diff or new U.S. approval claim is made. Safety text warns of blindness, tumors and infection from unapproved products and that trial registration is not marketing authorization. Older consumer pages must not be used as a complete current inventory of approved products. Targeted FDA warning-letter search produced no verified new relevant September item; this is a search limitation, not proof none exists. ### Health Canada Policy: https://www.canada.ca/en/health-canada/services/drugs-health-products/biologics-radiopharmaceuticals-genetic-therapies/applications-submissions/guidance-documents/cell-therapy-policy.html Advisory: https://recalls-rappels.canada.ca/en/alert-recall/health-canada-advising-canadians-about-potential-health-risks-associated-unauthorized Both retrieved directly. Autologous origin alone does not establish safety or exemption from drug regulation. The policy addresses narrow minimally manipulated/homologous lymphohematopoietic transplantation exceptions; other autologous cell products require appropriate authorization. The historical advisory warns of serious infections and unproven claims; its historical product count is not a current inventory. Targeted Canadian September advisory search did not verify a new cell-therapy policy change. No Canadian regulatory-status upgrade added. ### NIH/NCI and ClinicalTrials.gov NCI transplant overview: https://www.cancer.gov/about-cancer/treatment/types/stem-cell-transplant Web extraction succeeded. Established disease-specific blood-forming-cell transplantation remains distinct from general regeneration claims; transplant can restore hematopoiesis after intensive therapy, and donor immune effects can act against some cancers. Registry check described above remains investigational, not approval or outcome evidence. ### ISSCR and AAOS Guidelines: https://www.isscr.org/guidelines News: https://www.isscr.org/isscr-news Policy: https://www.isscr.org/stem-cell-policy Orthopedic guidance: https://orthoinfo.aaos.org/en/treatment/use-of-stem-cells-in-orthopaedics/ Guidelines extraction shows August 2025 version 1.2; targeted update concerns embryo models, with other sections remaining as published in 2021. News retrieved directly: September 17 items concern editorial appointments and a diabetic-kidney organoid model, not a patient cure. September 9 policy item concerns comments on FDA draft genome-editing development guidance; comments are not final regulation and predate this review window. Policy direct URLs returned 404, but web extraction returned policy text advocating oversight and opposing unproven treatment marketing. AAOS retrieved directly and via extraction; no orthopedic efficacy upgrade identified. ### Global regulators and medical tourism - EMA ATMP overview, direct retrieval succeeded: https://www.ema.europa.eu/en/human-regulatory-overview/advanced-therapy-medicinal-products-overview Maintains the distinction between regulated gene/cell/tissue-engineered therapies and unregulated offerings. No newly verified EU approval affecting this report. - MHRA ATMP licensing guidance, corrected URL directly retrieved after an obsolete collections URL returned 404: https://www.gov.uk/guidance/advanced-therapy-medicinal-products-regulation-and-licensing-in-uk Marketing authorization and clinical-trial oversight remain distinct from narrow unlicensed hospital-exemption/specials routes. No new weekly rule change verified. - PMDA framework directly retrieved, alongside the approval PDFs above: https://www.pmda.go.jp/english/review-services/reviews/0003.html - Singapore HSA supplement warning, web extraction succeeded after direct HTTP 403: https://www.hsa.gov.sg/announcements/false-and-misleading-stem-cell-claims-made-on-supplements/ Page displays April 6, 2026 and a migration notice. Oral placenta/stem-cell supplement cure claims are misleading; not a new September warning. - Australia TGA consumer guide, web extraction succeeded after direct timeout: https://www.tga.gov.au/news/news-articles/stem-cell-treatments-and-regulation-quick-guide-consumers Published September 11, 2019. Useful harm/medical-tourism context; historical "only approved" wording is not treated as a current comprehensive approvals audit. - Mexico/Panama/Cayman targeted September searches returned marketing and unrelated/low-quality items, not a new authoritative policy, reliable price change or major verified public outcome. No clinic recommendation, updated pricing claim or new testimonial was added. This is not a comprehensive review of local regulators' records. ## Wider literature screen and retained leads NCBI ESearch/EFetch retrieved the September 14-21 entry-date query for stem cells, MSCs, iPSCs, exosomes and hematopoietic transplantation combined with clinical trials, safety/adverse events, Parkinson's, diabetes, spinal cord, arthritis, autoimmune disease and cancer. The broad query returned 376 records, reconciled as 374 PubmedArticle records plus two PubmedBookArticle records. Titles were screened and selected clinical/review abstracts inspected; not every paper was read in full. Future issue dates and entry dates were not mistaken for new trial results. Additional source-grounded leads, not promoted into general clinical claims: - Bratt-Leal et al., Cell Stem Cell, September 17; PMID 42753738; DOI 10.1016/j.stem.2026.08.014. Genomic/transcriptomic quality control for autologous iPSC Parkinson's product: preclinical manufacturing, rodent efficacy and nine-month toxicology, NOT new human efficacy. Numerous author company employment/equity/patent interests disclosed. https://pubmed.ncbi.nlm.nih.gov/42753738/ - Ma et al., BMJ Open, September 16; PMID 42749369; DOI 10.1136/bmjopen-2025-115350. iPSC-derived exosomes for acute ischemic stroke: a phase I/II protocol, NOT results, despite efficacy wording in the title. https://pubmed.ncbi.nlm.nih.gov/42749369/ - PMID 42764516: observational European registry study of HSCT in late-onset metachromatic leukodystrophy. Stage at treatment matters; non-randomized comparison and progression still possible. Not a general neurologic cure. https://pubmed.ncbi.nlm.nih.gov/42764516/ - Kebir et al.; PMID 42735599; DOI 10.1016/j.msard.2026.107904; published September 9, newly indexed in the window. AHSCT versus alemtuzumab in highly active MS: observational comparisons, reconstructed time-to-event data, very-low-certainty conclusions. Do not infer proven comparative superiority or equivalent safety. https://pubmed.ncbi.nlm.nih.gov/42735599/ - Poutouri et al.; PMID 42737497; DOI 10.3390/ijms27177596; published August 25, newly indexed in the window. Frailty review: three heterogeneous RCTs, functional signals, no definitive efficacy conclusion; not proof of age reversal. https://pubmed.ncbi.nlm.nih.gov/42737497/ - Diabetes/kidney organoid and spinal-cord mouse studies are preclinical. Veterinary OA trial PMID 42761244 is in cats, not human arthritis evidence. Cancer-stem-cell biology papers are not necessarily therapeutic stem-cell transplant trials. No new general diabetes, spinal-cord, exosome, anti-aging or cancer-cure claim is justified by this screen. Local audit assets: research/health/stem-cells-weekly-evidence-2026-09-21/ contains retrieval status, directly fetched source text, PubMed query/results/XML and PMDA PDFs/text. Raw assets are retained locally rather than republished wholesale. ## Changes and non-changes - Added September 21 weekly subsection after the medical caution; original deep report and prior dated updates retained. - Clarified iPSC category and split Japan from the broader regional row with narrow, conditional indications and official links. - Updated article metadata, article sitemap lastmod, and added this source-note sitemap entry. - health.html, pricing, assets and styles unchanged: no new report or Health-page link was needed. - No new clinical recommendation, dosing, endorsement, broad cure claim or unsupported celebrity testimony. - Local and live pre-edit article/sitemap backed up before edits; directory-specific deployment is restricted to article, this note and sitemap on the existing Hostinger shared host. ## Next monitoring priorities Track larger retinal outcome cohorts and vision endpoints; confirm any future U.S. COPD registry/official regulatory document and peer-reviewed outcomes; follow Japan's required postmarketing evidence; distinguish MS biomarker analyses from randomized disability outcomes. Re-check FDA list with full access before making any new U.S. approval claim. Medical/editorial boundary: public-interest source review, not medical advice, diagnosis, treatment, dosing guidance, trial recruitment advice or medical-tourism endorsement.
