# Stem cells weekly source review - October 12, 2026 Decision: substantive public update warranted. New clinical implementation and survivorship evidence, plus two catch-up trials, merit a dated addition. No general cure claim or clinic endorsement. Access date: 2026-10-12 (PDT), obtained from the system clock. Latest existing review located: September 21. Search coverage therefore extends September 22-October 12; September findings are explicitly catch-up, not described as this week's publications. This is targeted monitoring, not a systematic review or an exhaustive regulatory audit. Article: https://managingexpectations.net/blog/articles/stem-cells-evidence-costs-risks-global-guide.html Previous review: https://managingexpectations.net/research/health/stem-cells-weekly-update-2026-09-21.md ## Selected findings and claim-to-source trail ### 1. Orca-T/Tregzi: clinical implementation this week; approval was in June Katie Shumake, Stanford Cancer Institute, "Stanford-developed therapy treats first patient after FDA approval," displayed October 7, 2026. https://med.stanford.edu/cancer/about/news/stanford-developed-therapy-treats-first-patient-after-fda-approv1.html Direct HTML and extracted text obtained. Visible date and article:published_time are October 7; JSON-LD datePublished is October 8. Use the visible date; do not infer a treatment date. Stanford reports the first commercial administration of Orca-T outside a trial. This is an institutional implementation report, NOT new outcome data on that recipient. Its goal-of-cure language is not evidence that the patient has been cured. FDA approval notice: https://www.fda.gov/drugs/resources-information-approved-drugs/fda-approves-allogeneic-regulatory-t-cell-based-immunotherapy-hspc-and-t-cells-vldq-use-matched Official page extracted. Approval date June 30, 2026. Exact scope: matched-donor hematopoietic stem-cell transplantation with a myeloablative preparative regimen, for hematopoietic and immunologic reconstitution and improved chronic GVHD-free survival in adults with hematologic malignancies. Precision-T randomized 187 adults. The estimated 12-month cumulative incidence of moderate-to-severe chronic GVHD was 12.6% versus 44.0%, comparing the complete study regimens, not an isolated component. FDA lists infection and other frequent adverse reactions, with warnings for graft failure, GVHD, infusion reactions, secondary/donor-origin malignancies and infectious-agent transmission. Registry independently retrieved: https://clinicaltrials.gov/study/NCT05316701 https://clinicaltrials.gov/api/v2/studies/NCT05316701 Phase 3, randomized, open-label, actual enrollment 187, results posted. Last update posted March 4, 2026. That registry date is not this week's news. The new event is Stanford's implementation report; the June approval is newly documented in this article. Label: approved for narrow indication. Do not generalize to unapproved regenerative infusions or availability in Canada. ### 2. Long-term transplant survivorship burden Mette Schaufuss Engedal et al. "Patient-Reported Late Effects in Allogeneic Hematopoietic Stem Cell Transplantation Survivorship: A Nationwide Population-Based Study." Transplantation and Cellular Therapy, electronic publication October 11, 2026. DOI: 10.1016/j.jtct.2026.09.052. PMID 42860152. https://pubmed.ncbi.nlm.nih.gov/42860152/ NCBI EFetch primary abstract retrieved. Nationwide Danish cross-sectional survey: 895 of 1,333 eligible survivors participated; median seven years after transplant. Fatigue 90%; 38% exceeded thresholds for clinical importance in at least five domains. Key passage: "substantial population-level burden with implications for long-term survivorship care." Interpretation: clinically important patient-reported outcome evidence, not a celebrity anecdote. Nonresponse, survivor selection, differing treatment eras and the cross-sectional design limit inference. These percentages do not show every symptom was caused by transplant and cannot be attributed to Tregzi or current products generally. No intervention recommendation is inferred. Label: observational human evidence. ### 3. Catch-up: Parkinson's trial did not demonstrate efficacy Maria Laura Gimeno et al. "A pilot phase IIa randomized, placebo-controlled, double-blind trial of intravenous human pluripotent adipose-derived stem cells (PASCs) in Parkinson's disease: The REPOSE study." Cell Transplantation, electronic publication September 23, 2026. DOI: 10.1177/09636897261492438. PMID 42775498. https://pubmed.ncbi.nlm.nih.gov/42775498/ NCBI abstract and conflict statement retrieved. Forty participants, placebo-controlled, rehabilitation in both groups, 12-month follow-up. Key passage: "Efficacy was not demonstrated in this pilot trial." None of seven exploratory endpoints met significance after false-discovery-rate adjustment. A thromboembolic event in the treatment group was adjudicated as of uncertain relationship; a placebo-group death was adjudicated unrelated. Do not convert the abstract's favorable tolerability conclusion into proof of rare-event safety. Manufacturer/sponsor affiliations and investigator service fees are disclosed. The study does not validate commercial Parkinson's infusions. This product is distinct from Japan's iPSC-derived dopaminergic progenitors. Label: under study; efficacy not demonstrated. ### 4. Catch-up: small randomized diabetes signal, not a cure "Safety and efficacy of human gingiva-derived mesenchymal stem cells in patients with type 1 diabetes: a double-blind, randomized, placebo-controlled, phase 1 trial." Signal Transduction and Targeted Therapy 11, 434, September 30, 2026. DOI: 10.1038/s41392-026-03021-8. PMID 42816465. Trial identifier ChiCTR1800018339. https://pubmed.ncbi.nlm.nih.gov/42816465/ https://www.nature.com/articles/s41392-026-03021-8 NCBI EFetch abstract verified; publisher extractor returned mainly references, so interpretation is abstract-level, not a full methodological audit. Eighteen randomized participants completed the trial. Authors report improved residual beta-cell function/C-peptide and reduced insulin requirements at 12 weeks, with equal glycemic control between groups. One participant's insulin-withdrawal report is not used as cure evidence. Small sample and short assessment period cannot establish durability or uncommon harms. Gingiva-derived MSC immune-modulation is not the same technology as stem-cell-derived islet replacement. Label: promising/under study. No treatment changes or dosing instructions. ## Required source checks and unchanged conclusions ### FDA safety and approvals - Approved list: https://www.fda.gov/vaccines-blood-biologics/cellular-gene-therapy-products/approved-cellular-and-gene-therapy-products - Consumer warning: https://www.fda.gov/vaccines-blood-biologics/consumers-biologics/important-patient-and-consumer-information-about-regenerative-medicine-therapies - Current-year safety index: https://www.fda.gov/vaccines-blood-biologics/safety-availability-biologics/2026-safety-and-availability-communications - Human-cell/tissue warning: https://www.fda.gov/vaccines-blood-biologics/safety-availability-biologics/patient-and-consumer-warning-about-potential-serious-risks-harm-following-use-unapproved-products All extracted. List includes Tregzi; product-specific approval notice used rather than a page-date change. Consumer page is dated June 3, 2021; its historic approved-product inventory must not override today's list. Current-year index includes October 6 immunoglobulin notices, not a new general stem-cell authorization. Human-cell/tissue warning describes reports including a death after an unapproved placental product; it notes testing did not isolate the associated pathogen. Do not claim confirmed causation or call this an October warning. Existing unapproved-product cautions retained. ### Health Canada - Policy: https://www.canada.ca/en/health-canada/services/drugs-health-products/biologics-radiopharmaceuticals-genetic-therapies/applications-submissions/guidance-documents/cell-therapy-policy.html - Advisory: https://recalls-rappels.canada.ca/en/alert-recall/health-canada-advising-canadians-about-potential-health-risks-associated-unauthorized Both directly retrieved, HTTP 200, after alternate guessed URLs failed. Own-cell origin does not establish safety or regulatory exemption. The advisory is dated May 15, 2019; its product count is historical. Targeted current searches did not verify a new relevant Canadian advisory or cell-specific policy change in the review window. Broader clinical-trial and reliance-framework search hits were not treated as authorization of particular therapies. No Canadian approval claim added. ### NIH/NCI and ClinicalTrials.gov NCI: https://www.cancer.gov/about-cancer/treatment/types/stem-cell-transplant Extracted. Established transplantation for selected blood cancers/disorders remains high-intensity specialist medicine, distinct from general regeneration marketing. COPD registry rechecked directly: https://clinicaltrials.gov/study/NCT06986070 https://clinicaltrials.gov/api/v2/studies/NCT06986070 Last update August 11, phase 1, open-label/nonrandomized, recruiting, estimated enrollment 18, hasResults=false. No new posted outcomes; September's company-reported U.S. permission-to-study announcement was not upgraded to marketing approval. Precision-T check is described above. ### ISSCR and orthopedics - https://www.isscr.org/guidelines - https://www.isscr.org/isscr-news - https://www.isscr.org/stem-cell-policy - https://orthoinfo.aaos.org/en/treatment/use-of-stem-cells-in-orthopaedics/ (redirects to https://www.orthoinfo.org/treatment/use-of-stem-cells-in-orthopaedics) Extracted. ISSCR guideline page still identifies August 2025 version 1.2, a focused embryo-model revision; other sections remain from 2021. News/policy pages emphasize oversight and evidence. October symposium/award notices are not clinical results or policy authorization. AAOS continues to describe limited/mixed orthopedic effectiveness evidence. No efficacy upgrade or arthritis cure claim added. ### Global regulation and medical tourism - EMA warning, March 13, 2025: https://www.ema.europa.eu/en/news/unregulated-advanced-therapy-medicinal-products-pose-serious-risks-health Extracted; contamination, composition and unproven-benefit concerns remain relevant. Not a new October warning. - MHRA: https://www.gov.uk/guidance/advanced-therapy-medicinal-products-regulation-and-licensing-in-uk Direct HTTP 200 and extraction. Last update September 11, 2026 adds British Pharmacopoeia scientific-advice information; predates previous review. No new weekly approval/rule change inferred. - PMDA framework: https://www.pmda.go.jp/english/review-services/reviews/0003.html - PMDA translated review index: https://www.pmda.go.jp/english/review-services/reviews/approved-information/0004.html Retrieved. AMCHEPRY and RiHEART remain March 2026 entries. Existing conditional-approval discussion retained. Translated-report index is selective, not a complete current regulatory audit. - Singapore HSA: https://www.hsa.gov.sg/ctgtp/overview/regulatory-overview/ Extracted, last updated April 14, 2026. Distinguishes risk classes and exclusions; culture-expanded cells fall within higher-risk Class 2. No new weekly change established. - Australia TGA: https://www.tga.gov.au/news/news-articles/stem-cell-treatments-and-regulation-quick-guide-consumers Direct request timed out; extraction succeeded. Historical September 11, 2019 consumer guide; useful risk context, not a current approvals inventory. - Mexico/Panama/Cayman: targeted current searches returned clinic/comparison marketing and accounts of older CDC cases, not a newly verified regulator action, comparable price change or independently documented newsworthy patient outcome. No clinic recommendation or new price estimate. This does not certify the absence of local changes. A STAT October 5 policy-advocacy story was located but full text was paywalled: https://www.statnews.com/2026/10/05/stem-cell-industry-seeks-fda-regulation-like-peptides Retained as a reporting lead only. Advocacy is not a change in law; no claims from the inaccessible story added. ## Literature screen and non-selected leads NCBI ESearch/EFetch queried stem cells, MSCs, iPSCs, exosomes and hematopoietic transplantation with trial/safety/adverse-event/Parkinson's/diabetes/spinal-cord/arthritis/autoimmune/cancer terms, entry dates September 22-October 12 inclusive. Returned 976 records; XML contains 974 PubmedArticle records plus two PubmedBookArticle records. Automated title/abstract filtering followed by targeted clinical abstract inspection; not every paper was read. Entry dates and publication dates are separate. - PMID 42807397: six-person chronic lung allograft dysfunction feasibility cohort, up to 58 months follow-up. Authors explicitly say lung-function findings are exploratory, not efficacy evidence; underpowered for meaningful infection-risk differences. Not COPD cure evidence. - PMID 42798182: investigational HSC gene therapy for MPSIIIA, 80-month follow-up in one patient. Important rare-disease lead, not general neurologic proof. - PMID 42823558: German CAR-T/autoSCT cost study, September 30. Historical insurer data; excludes CAR-T drug acquisition and discloses industry sponsorship. Not a 2026 patient cash price; no replacement of existing price ranges justified. - PMID 42803164: Crohn's fistula meta-analysis searches only through September 2024; pooled healing proportions are not by themselves randomized comparative efficacy or current product-approval evidence. No indication upgrade. - Preclinical exosome/spinal-cord, Parkinson's and diabetic-wound studies remain animal/cell-model evidence. Cancer-stem-cell biology is not automatically therapeutic transplantation. No new celebrity testimony meets the publication threshold. ## Article edits and deployment scope Added October 12 subsection immediately after medical caution, with original deep report and all prior updates intact. Updated article review date and added a product-specific Tregzi clarification to the existing U.S. regulation row. Article/source-note sitemap dates updated. Health page, pricing, assets, styles and other public files remain unchanged; no new report link was needed. Local pre-edit article, sitemap and Health page backed up in _local-backups/stem-cells-2026-10-12/. Live article was compared with local source before edits and matched after UTF-8 decoding. Deploy only article, this note and sitemap to existing Hostinger shared hosting after remote backups; no DNS, VPS or configuration changes. Local audit assets: research/health/stem-cells-weekly-evidence-2026-10-12/ (query, XML, parsed abstracts, direct page snapshots, registry JSON and retrieval status). These raw assets remain local. Script: _work/stem_monitor_20261012.py. Deployment verification is recorded locally, not represented as complete merely because this note exists.
